Who is eligible to apply? How are award amounts determined?

Eligibility Requirements:

• • Applicants must be residents of the United States;
  • Applicants must be at least 18 years of age;
  • This award is specifically for a pet acquiring their initial case review only. It cannot be utilized for any subsequent case reviews pertaining to the same malpractice/negligence incident.

Please note that only one application will be accepted per case. One pet per application. If you have more than one pet applying, please submit them separately. All financial determinations made are final and adhere to the established guidelines. Awards are allocated on a first-come, first-served basis and are subject to the availability of funds at the time of application.

Income Guidelines:

Award amounts are based on Federal Proverty Level Guidelines and are based on the number of household members and the total gross income(s) for all adults in the home.

• • If your household income is  0 – 10% of Federal poverty levels, award amount is $650.
  • If your household income is 11 – 20% of Federal poverty levels, award amount is $600.
  • If your household income is 21 – 30% of Federal poverty levels, award amount is $550.
  • If your household income is 31 – 40% of Federal poverty levels, award amount is $500.
  • If your household income is 41 – 50% of Federal poverty levels, award amount is $450.
  • If your household income is 51 – 60% of Federal poverty levels, award amount is $400.
  • If your household income is 61 – 70% of Federal poverty levels, award amount is $350.
  • If your household income is 71 – 80% of Federal poverty levels, award amount is $300.
  • If your household income is 81 – 90% of Federal poverty levels, award amount is $250.
  • If your household income is 91 – 100% of Federal poverty levels, award amount is $200.

This documentation must clearly indicate the individual filing the tax return, the tax year, and the total gross income from all sources. Please redact or hide social security numbers, as this information is not required. Failure to redact is at the applicant’s own risk.
